Scaling and Root Planing for Gum Disease

Scaling and root planing is a deep cleaning below the gumline used to treat gum disease.

Why Do I Need It?
Gum disease is caused by a sticky film of bacteria called plaque. Plaque is always forming on your teeth, but if they aren’t cleaned well, the bacteria in plaque can cause your gums to become inflamed. When this happens, your gums will pull away from your teeth and form spaces called pockets. Plaque then gets trapped in these pockets and cannot be removed with regular brushing. If untreated, gum disease could lead to bone and tooth loss.

If gum disease is caught early and hasn’t damaged the structures below the gum line, a professional cleaning should do. If the pockets between your gums and teeth are too deep, however, scaling and root planing may be needed.

A July 2015 study in the Journal of the American Dental Association finds that scaling and root planing is beneficial to patients with chronic periodontitis (gum disease that has advanced past gingivitis). Chronic periodontitis affects 47.2% of adults over 30 in the United States.

What Happens During Scaling and Root Planing?
This deep cleaning has two parts. Scaling is when your dentist removes all the plaque and tartar (hardened plaque) above and below the gumline, making sure to clean all the way down to the bottom of the pocket. Your dentist will then begin root planing, smoothing out your teeth roots to help your gums reattach to your teeth. Scaling and root planing may take more than one visit to complete and may require a local anesthetic.

After Care Tips
After a deep cleaning, you may have pain for a day or two and teeth sensitivity for up to a week. Your gums also may be swollen, feel tender and bleed.

To prevent infection, control pain or help you heal, your dentist may prescribe a pill or mouth rinse. Your dentist may also insert medication (subantimicrobial-dose doxycycline) directly into the pocket that was cleaned.

Your dentist will schedule another visit to see how your gums have healed and measure the depth of your pockets. If they have gotten deeper, more treatment may be needed.

Good dental care at home is essential to help keep gum disease from becoming more serious or recurring.  Brush your teeth twice a day with a soft brush, clean between your teeth daily, eat a balanced diet, avoid using tobacco and see your dentist regularly.

Water Flossing

Water flossing is a way to clean between and around your teeth. A water flosser is a handheld device that sprays streams of water in steady pulses. The water, like traditional floss, removes food from between teeth. 
Water flossers that have earned the ADA Seal of Acceptance have been tested to be safe and effective at removing a sticky film called plaque, which puts you at a higher risk for cavities and gum disease. Water flossers with the ADA Seal can also help reduce gingivitis, the early form of gum disease, throughout your mouth and between your teeth. Get a list of all ADA-Accepted water flossers. 
Water flossers can be an option for people who have trouble flossing by hand. People who have had dental work that makes flossing difficult—like braces, or permanent or fixed bridges—also might try water flossers. 
Cleaning between your teeth once a day is an important part of your dental hygiene routine. You should also brush your teeth twice a day for two minutes and see your dentist regularly.

New Year, Healthier Mouth

What does ringing in the new year have to do with being mouth healthy? 


More than you may think. Did you know that you should replace your toothbrush every three to four months? Bristles that become frayed and worn are less effective at cleaning your teeth. That means, celebrating the new year with a brand new toothbrush is actually smart dental hygiene.


Here are MouthHealthy resolutions:

  • Start brushing 2min2x. Always brush twice a day for two minutes for healthier teeth, good breath, fewer cavities, and to avoid painful dental problems. 
  • Floss daily. Flossing is part of being mouth healthy.
  • Chew sugarless gum. Chewing sugarless gum for 20 minutes following meals can help prevent tooth decay.
  • Eat a healthy diet. Eat a balanced diet and limit between-meal snacks.
  • Drink fluoridated water. Fluoride helps prevent cavities by making teeth more resistant to the acid attacks that cause cavities.
  • See your dentist. Regular dental visits will help you be Mouth Healthy for Life.

Cavity Fillings: What to Expect, Types & Potential Problems

The process of filling cavities is a fairly simple and straightforward one that can be done right at your dentist's office.

Filling Cavities: What to Expect

You should expect to be at your dentist's office for around an hour. This gives him or her enough time to take x-rays if needed, talk to you about the procedure and complete the dental work. Before filling cavities, your dentist will numb your teeth, gums and surrounding skin to avoid and lessen discomfort during the procedure. Next, he or she will drill out the decay in the tooth and replace it with a filling. This process only takes a few minutes.
Once you're done, your mouth will probably remain numb for a few more hours. There aren’t any significant risks associated with filling cavities, but be sure to keep your dentist’s contact information on hand in case you have any questions or complications.
The most common use of tooth fillings is to fill a cavity in the tooth. But tooth fillings also can be used to repair damage to teeth caused by teeth grinding (bruxism) or to replace part of a broken tooth.

Types of Cavity Fillings

Many options are available for tooth fillings, and all of them have their pros and cons. Types of tooth fillings include gold, silver amalgam (a composite of mercury, silver, and other metals), tooth-colored composite material, porcelain, and a special type of glass. The best tooth fillings for you will depend on cost, what your insurance may cover, and your aesthetic preferences.
There is a wide variety of materials used for filling cavities and they vary in strength and color. The two most common types are amalgam and composite.
  • Amalgam Fillings: Amalgam has been used by dental professionals for more than a century; it is the most researched material used for filling cavities. Amalgam fillings are strong and are therefore ideal for filling cavities in the back of the mouth such as in the molars, where chewing takes place. Since they are made of a combination of several metallic elements, amalgam fillings can be noticeable when you laugh or smile. These fillings are among the least expensive of all cavity-filling materials.
  • Composite Fillings: Sometimes referred to as composites or filled resins, these fillings feature a combination of glass or quartz filler and can be made to match the color of your tooth. Composite fillings are also fairly durable and are ideal for small-to-mid-size restorations in areas of your mouth that perform moderate chewing.
  • Metals: Gold or silver amalgam are the most common metals used for a cavity filling. Gold fillings can cost as much as 10 times more than silver amalgam fillings, but some people prefer the appearance of gold to silver fillings if they want the durability of metal vs. a less-durable composite material. Some people don’t like the appearance of metal fillings, but metal fillings can last as long as 10-15 years before they need to be replaced.
  • Ceramic: A ceramic cavity filling (usually made of porcelain) is tooth-colored, and it may be less likely to show tooth stains over time than a composite cavity filling. But price is a factor—a ceramic filling can be nearly as expensive as a gold cavity filling.
  • Glass Ionomer: This blend of acrylic and glass is used to create a cavity filling that releases fluoride to help protect teeth. But a glass ionomer cavity filling is less durable than other types, and may need to be replaced in as little as five years.

Taking Care of Cavity Fillings

You may experience some sensitivity and pain after receiving tooth fillings, but this discomfort should subside. Don't neglect your oral care routine. Instead, try products designed specifically to protect sensitive teeth. When to Replace a Cavity Filling

Tooth fillings usually last for many years before they need to be replaced. But tooth fillings can wear out over years of chewing. If you clench or grind your teeth, you may need to have tooth fillings replaced sooner.
If you notice signs of wear on your tooth fillings, such as cracks or worn areas, see your dentist to have the filling replaced as soon as possible. Continuing to chew with a damaged filling can cause the tooth to crack and require additional repair that is more expensive and more complicated than a simple cavity filling. If additional tooth decay develops around a filling, whether or not the filling is damaged, your dentist may choose to repair the tooth with a crown instead of a second cavity filling.

Other Potential Problems with Cavity Fillings

It’s important to know about potential problems, so you can see your dentist promptly to have cavity fillings adjusted or repaired. Possible complications from cavity fillings include: 
  • Infection: Sometimes a cavity filling will pull away from the tooth to which it is attached, creating a small space. This space can be a breeding ground for bacteria that can cause additional tooth decay. If you notice a space between your tooth and your cavity filling, visit a dentist as soon as possible.
  • Damage: Sometimes a cavity filling breaks, cracks, or falls out. Damage to a filling can occur when you bite down on something hard or if you are hit in the mouth while playing sports. See a dentist as soon as you notice damage to a cavity filling to avoid irritation and infection of the unprotected tooth.

How To Limit The Effects Of Sugar On Teeth




Cookies, cakes, candies and sodas – everywhere you go, there are sugary treats to tempt you and your kids. The effects of sugar on teeth may not be noticeable right away, but too much can lead to tooth decay if you don't stay on top of it. Here's how sugar can harm your family's dental health and what you can do to prevent it.

Acid Attacks

When you eat or drink sugary foods – refined, processed or in the form of carbohydrates – you're feeding the beast. Bacteria in your mouth digest the foods you eat and specifically feed on the sugar, producing acids that can slowly dissolve tooth enamel and cause tooth decay. According to the National Institute of Dental and Craniofacial Research (NIDCR), these acids do the most damage to your teeth for 20 minutes after eating; this is what is known as an "acid attack." So the more sugary foods you eat throughout the day, the more your teeth are exposed to decay-causing acids.

Sensible Food Choices

What can you do to protect your family from these ubiquitous acid attacks? Eliminating sugars completely from your family's diet is unrealistic, because most of your foods contain some form of sugar or starch. However, you can still control the amount of sugar you and your family consumes and reduce the total time your teeth are susceptible to bacterial acids. Consider the following dietary adjustments:

  • Eat a nutritious and balanced diet of grains, fruits, vegetables, protein and low-fat dairy.
  • Check food labels for "hidden" sugars.
  • Limit between-meal snacks.
  • Avoid sugary foods that stay in your mouth for a long time, like hard, sticky or chewy varieties of candy.
  • Offer healthy snacks, such as cheese, fruits, yogurt, peanut butter, chocolate milk and sugarless gum.
  • Save your sugary treats for the end of a meal or before a tooth-brushing session, rather than throughout the day.
  • Substitute water for sugar-laden sodas and fruit juices.

Oral Hygiene Is a Good Defense

Brushing your teeth thoroughly after eating cleans recent sugary foods and existing bacteria off of your teeth, and flossing once a day will clean between your teeth – a prime spot for bacteria to hide. Preventive Care and Early Detection

There is no denying that people love their sugar. In fact, the NIDCR suggests the average person in the U.S. eats about 147 pounds of it a year – that produces an enormous potential for tooth decay. Make the necessary changes in diet and dental hygiene now, so the effects of sugar on teeth don't come when your family is preoccupied with bigger life events.

All About Cavities

What's in Your Mouth? 
To understand what happens when your teeth decay, it's helpful to know what's in your mouth naturally. Here are a few of the elements: 

  • Saliva - Your mouth and teeth are constantly bathed in saliva. We never give much thought to our spit, but this fluid is remarkable for what it does to help protect our oral health. Saliva keeps teeth and other parts of your mouth moist and washes away bits of food. Saliva contains minerals that strengthen teeth. It includes buffering agents. They reduce the levels of acid that can decay teeth. Saliva also protects against some viruses and bacteria. 
  • Plaque - Plaque is a soft, gooey substance that sticks to the teeth a bit like jam sticks to a spoon. Like the slime that clings to the bottom of a swimming pool, plaque is a type of biofilm. It contains large numbers of closely packed bacteria, components taken from saliva, and bits of food. Also in the mix are bacterial byproducts and white blood cells. Plaque grows when bacteria attach to the tooth and begin to multiply. Plaque starts forming right after a tooth is cleaned. Within an hour, there's enough to measure. As time goes on, the plaque thickens. Within two to six hours, the plaque teems with bacteria that can cause cavities and periodontal (gum) disease. 
  • Calculus - If left alone long enough, plaque absorbs minerals from saliva. These minerals form crystals and harden into calculus. Then new plaque forms on top of existing calculus. This new layer can also become hard. 
  • Bacteria - We have many types of bacteria in our mouths. Some bacteria are good; they help control destructive bacteria. When it comes to decay, Streptococcus mutans and Lactobacilli are the bacteria that cause the most damage to teeth.

Oral Cancer

In Canada, 3400 new cases of mouth cancer are diagnosed each year. About 50% of those diagnosed do not live longer than 5 years after diagnosis because it wasn’t detected early enough.
The most common sites for oral cancer to be found are the tongue (which has the highest prevalence), throat, floor of the mouth and lips. Regular tobacco use (both chewing and smoking), alcohol consumption and prolonged sun exposure all increase risk in addition to age.
